The game takes players back to the home place of the original game years ago, circa the early 1900s.
Hangar 13, developers of Mafia III, are developing Mafia: The Old Country as a prequel to the Mafia franchise, which began in 2002. It will focus on the twisted history of organized crime.
Mafia: The Old Country will debut August 8, 2025. This will give players a jump start on their summer. It is confirmed for release on PlayStation 5, Xbox Series X/S, Windows, and, most recently, GeForce Now. This could allow those who play via cloud gaming a way to stream it.

Players will take on the role of Enzo Favara. He is a young Sicilian who embarks on a life of violent crime after having a rough existence as a miner in a sulfur mine. The game is set in San Celeste, a fictional Sicilian town shown in Mafia II, during the rise of fascism under Benito Mussolini.
As Enzo becomes associated with the Torrisi crime family in the game, it explores the heavy cost of power. It also examines the conflict of loyalty and survival.

The game, unlike Mafia III, is a linear, story-driven game. It is similar to both Mafia and Mafia II. The gameplay is third-person and set in built environments. The characters and plotlines are well-developed. The game features choice-driven narratives, meaning every choice matters in the outcome. This makes each player's experience more personal.
The game is being developed with Unreal Engine 5. This is producing beautiful environments. Hangar 13 holds visuals to a high standard for historical accuracy and atmosphere. While the game includes full voice dubbing in six languages, including English and German, Italian voice acting is missing. Instead, Hangar 13 introduced the Sicilian dialect of dubbing. They explained this is "essential to the authenticity of the game."
