After a person died from a heart attack, a family in the United States was left speechless by the enormous medical cost of $195,000 for barely four hours in the intensive care unit. 

The family was solely responsible for the expense, as the patient’s insurance had expired two months before the occurrence. 

Instead of just paying the bill, the family asked for it to be itemized and discovered that most of the expenses were grouped under ambiguous headings, such as ‘Cardiology - $70,000.’

They then used a subscription to the AI chatbot Claude at US$20 to help them with the billing breakdown.

AI Uncovers Duplicate, Mis-Coded Hospital Charges

The AI model reviewed the entire bill along with procedure codes (CPT/ICD) and applied standard billing norms. It highlighted the following key issues:

  • Duplicate Billing: The hospital charged a ‘master procedure’ and simultaneously billed each part of the procedure separately. This alone was said to contribute to the charges, with a whopping $100,000.

  • Mis-Coding: The stay was classified as an inpatient admission, which is costly, rather than as an emergency department visit or a short stay.

  • Regulatory/Billing Red Flags: For instance, the ventilator service was billed on the same day as admission, although coding rules may disallow bundling of that service. 

The family constructed a formal dispute letter to the hospital billing office, citing the AI’s findings. The letter pointed out specific duplicative charges, improper coding, and potential legal exposure, as well as public scrutiny, effectively pressuring the hospital into negotiations. 

Can AI Be Trusted with Such Decisions?

The AI instrument served as a dual assistant for the entire dispute, providing both auditing and drafting assistance. Consequently, the hospital agreed to reduce the bill to $33,000.

Although considerable, this amount indicated a reduction of more than 80% from the starting sum. Over time, the family referred to a $20/month AI membership as ‘the greatest investment’ they had made, given the circumstances. 

There are some warnings, nevertheless, that come with the report: the narrative was initially reported by a platform user on Threads (the handle is ‘nthmonkey’), and the hospital has not authenticated the story itself. 

The main impact is two-fold. To start with, US medical billing is highly opaque and difficult to understand for those outside the medical field.
